BIRTH CERTIFICATE BEFORE REGISTRATION CAN BE COMPLETED
The Player's birth certificate must be scanned (document scanning can be done at OfficeMax, Office Depot, Staples, UPS Stores, etc... ) and emailed to: Kay Patterson at rosemontregistrar@att.net
A current photo of the Player must be uploaded during the online registration process. The photo will be used on the player pass and must be a headshot only! No hats or sunglasses!
When registering online, the Player Registration form (1601) and the Parent Ethics form will be printed by the RSC Registrar who will provide them to your child's coach in order to have you sign them. These forms must be signed at the first practice or team meeting.
If these two forms are not signed, the player will not be allowed to play.
